There is a computer program to help you prepare a provisional custody by mandate form provided by Louisiana Revised Statutes 9:954. Here is the language from that statute that says who may use the form set out in RS 9:954: "The following is a suggested form that may be used by a person having parental authority to delegate the provisional custody of the named child:" Use this link to get to the online program to create your form: [May 2021 New link pending.]

There is a computer program to help you prepare a provisional custody by mandate form provided by Louisiana Revised Statutes 9:962. Here is the language from that statute that says who may use the form set out in RS 9:962: "The following is a suggested form that may be used by a natural tutor or cotutors with custody, but not yet judicially qualified, or a grandparent awarded custody of a child to delegate to any natural person the custody of the named child:" Use this link to get to the online program to create your form: [May 2021 New link pending.]

NOTE: When the computer program creates your form you may need to adjust the size or "zoom" of the document to make sure the format is correct and to be sure that you can see the signature lines. Read your form carefully when you get it from the computer. Look to see if your form has areas where you need to circle a choice, check a checkbox, or fill in other information, such as a Social Security Number. Check forms for places where it must be signed, dated, or notarized, or where a court case number or other information must be written in.
