The computer program will help you create forms for a small claim in a Louisiana Justice of the Peace Court.
Some of the online forms programs may work only in certain browsers. Try Chrome or another browser if you have difficulty with getting an online forms program to open in the browser you are using.The computer program does not cover forms for a small claim in a City Court or other court that is not a Justice of the Peace Court. Check with the Louisiana District Court for the district and parish where you plan to file to find out where the small claims court is. The small claims court may also have its own forms for you to fill out. This site may help locate the Justice of the Peace for a given place in Louisiana: https://voterportal.sos.la.gov/ElectedOfficials
If your case is not in Justice of the Peace Court, you may want to contact the clerk of court for the court you need to see if they have sample forms or fill-in-the blank forms you can use.

NOTE: When the computer program creates your form you may need to adjust the size or "zoom" of the document to make sure the format is correct and to be sure that you can see the signature lines. Read your form carefully when you get it from the computer. Look to see if your form has areas where you need to circle a choice, check a checkbox, or fill in other information, such as a Social Security Number. Check forms for places where it must be signed, dated, or notarized, or where a court case number or other information must be written in.
